A mobile health industry grows in San Diego

In one of this week's top stories, mHealth Initiative founder Peter Waegemann talks about the need for "an m-health community." He was talking about a community in the virtual sense, not in the geographic sense, but there is one locale that's positioning itself as a leader in mobile healthcare: San Diego.

While FierceMobileHealthcare took a publishing holiday last week, I was busy having some interesting conversations with various movers and shakers in mobile healthcare and in health IT in general. Boston-based Waegemann was one such individual. There are some others that I'll talk about in FierceHealthIT and FierceEMR over the next couple of weeks.

I also heard from Paul Sonnier, co-chair of the Healthcare Communications Special Interest Group at CommNexus San Diego, a local network of telecommunications companies. Sonnier started a wireless health group on networking site LinkedIn that originally was intended for just those in the San Diego area. But he's since taken the geographic notation off the group's title, and now invites others to join.

1. Telehealth poised for growth, EHR integration in 2010

With the passage of the American Recovery and Reinvestment Act last February, this is supposed to be the year healthcare providers start to adopt electronic health records en masse. But, says Computerworld, the EMR effort means "2010 could be the year telehealth technology finally allows doctors to monitor their patients' health wirelessly in real time--no matter where the patient is."

The magazine bases that conclusion in part on a recent report from Accenture that says consumer health electronics, predictive modeling and low-cost Internet connectivity to enable better decision making will transform chronic disease care. The story also quotes an Ingenix Consulting executive, who believes EHRs will drive growth in wireless monitoring technologies.

"One big benefit of EHRs is the e-visit," says Ingenix Consulting Managing Director Larry Leisure. "Imagine a patient and doctor having an email conversation with bio-monitoring equipment transmitting data. They can have a conversation with shared information available to both. It enables patients and physicians to have a different relationship. Think about the cost avoidance in that."

Here at FierceMobileHealthcare, we believe EHRs won't start to show their full potential to improve the quality and efficiency of care until medical devices can automatically populate patient records and trigger clinical decision support. We take issue with Computerworld's characterization of Google Health and Microsoft HealthVault as "online EHRs," though.

2. Wireless home monitoring likely to become mainstream

The electronic pendant around the neck of 88-year-old Grif Crawford is more than just a panic button. It's a two-way communicator that can remind Crawford to take his medicine, answer his phone, or let him know if there's a fire. And yes, it still can call for help in an emergency.

This new generation of health monitors may cost $150 to $200 a month right now, but what is the value if the technology lets seniors remain in their homes longer and avoid costly nursing homes and assisted-living facilities? People are just beginning to ask that question. "We're at the beginning of the wave," Elinor Ginzler, senior vice president for livable communities for AARP, says in an Associated Press story. "Money is an issue."

As awareness increases and prices fall, though, this technology is headed for the mainstream, Ginzler suggests.

3. iPhone app tracks high-acuity cases for harried docs

There are interesting iPhone apps that are fun to play around with, and then there are iPhone apps that serve a clear clinical purpose. Vigilance is the latter.

The Vigilance app, developed at Vanderbilt University Medical Center and marketed by Birmingham, Ala.-based Acuitec, tracks patient vital signs, transmits live video from exam and operating rooms and sends alerts when patients are in distress so doctors can respond even before nurses page them. "I can be more on top of what's happening," Vanderbilit anesthesiologist Dr. Paul St. Jacques tells the Birmingham News. "When I get into a room, frequently people will say, 'I'm so glad you're here,' or 'I was just about to call you.'"

St. Jacques helped create Vigilance--originally on the PC--because he and his colleagues often administer anesthesia to as many as four patients simultaneously. "It's difficult to know what's going on in one O.R. if you're standing in the next one," he explains.

Acuitec, which acquired the marketing license for Vigilance in 2008, has adapted the aggregation software for use in emergency departments and in intensive care units. The iPhone app came along last fall. Vanderbilit plans on using the app to monitor cases in the African nation of Botswana. Acuitec CEO Lionel Tehini sees value in tracking patients in rural areas of America, too.

4. Waegemann sees huge potential, but much fragmentation in mobile health

Yet another sign that the field of mobile healthcare is maturing was last year's decision by Peter Waegemann to shut down the Medical Records Institute and related TEPR (Toward an Electronic Patient Record) conference after 25 years and transition to a new organization called the mHealth Initiative. The Medical Records Institute previously had projects called the Mobile Healthcare Alliance and, more recently, the Center for Cell Phone Applications in Health Care.

"I'm excited about m-health the way I was about EMRs eight years ago," Waegemann tells FierceMobileHealthcare. He sees more innovation in mobile healthcare right now than in other areas of health IT, and nearly unlimited potential for mobile technologies to improve patient care and lower healthcare costs. But, as with EMRs not too long ago, mobile health is a rather fragmented field. "We think we can create substantial benefits," Waegemann says, "but there needs to be an m-health community."

"There are still too many Band-Aids," Waegemann adds.

Right now, Waegemann is trying to get his arms around the scope of the m-health field. The group has identified 12 application clusters and Waegemann hopes to define each area by the end of 2012. The mHealth Alliance holds its first International mHealth Networking & Web Conference next month in Washington, D.C., and Waegemann also is looking for grant funding to support projects to test mobile technologies in the real world.

5. IBM, Continua to showcase wireless interoperability at CES

The annual International CES (formerly the Consumer Electronics Show) opens Thursday in Las Vegas. Normally we wouldn't care all that much, given the consumer focus, but this time, the huge show features a first-ever Digital Health Summit. We'd be there, but the organizers didn't publish an agenda for the health portion until it was too late to find an airfare that wasn't prohibitively expensive. Fortunately, we're already getting a decent preview from some of the vendors.

IBM and Vignet, a Virginia-based seller of connected health products, both will demonstrate a home-based pulse oximeter that connects to the Internet to notify health professionals when a patient's blood oxygen level is low, according to a PC World report.

Both firms, as members of the Continua Health Alliance, also will take part in a larger demonstration of end-to-end connectivity based on Continua's standards-based architecture. The plan sounds similar to the Integrating the Healthcare Enterprise demo at the annual HIMSS conference.

For this demonstration, a Nonin Medical wireless pulse oximeter will send data via Bluetooth to a computer running the Vignet Connected Health Services platform. The PC will then upload the data to an IBM server, following a draft Continua WAN interface standard. The server should be able to share the data securely with healthcare providers, disease management companies, electronic health records and personal health records.

Amcom Software, an Eden Prairie, Minn.-based maker of call-center and emergency notification software, is banking on mobile healthcare to provide returns to two new venture investors. CEO Chris Heim sees a "tidal wave related to mobile healthcare" and strong demand for "unified communications" within hospitals and health systems to continue a growth track that has seen annual revenues soar to $33.7 million in March 2009 from $11.8 million just two years earlier. 4. Buy advertising space on discussion board web
sites. They are usually arranged by subject; that
makes them highly targeted.


5. Start a free e-mail newsletter to create your own
opt in list. Create a title that grabs readers attention.
Submit it to free e-zine directories on the internet.


6. Record all your new promotion ideas into an idea
journal, good or bad. Sometimes you can combine
ideas to create new ones to increase your sales.


7. Convert your web site and free e-mail newsletter
into different languages. This will increase your over-
all target market.


8. Make your classified ads stand out in a crowd.
Use all capitol letters in the headline, divide letters
with extra spaces, add in text symbols, etc.


9. Remember your customer is always right, even
if they are not. Resolve all conflicts quickly and
painlessly. They are the lifeblood of your business.


10. Create a bond with your visitors by bringing up
likes or dislikes you have in common with them in
your ad copy. Just make sure you do your research.

    "Christmas and Holiday Season Ecommerce Sales Up 5 Percent"

    Reporting on retail ecommerce sales during the Christmas holiday
    season (November 1 to Christmas Eve), comScore estimates that
    about $27 billion was spent online, 5 percent more than during
    the same period in 2008.

    From Black Friday (the day after Thanksgiving Day in the U.S.)
    to Christmas Eve - and after adjusting for the additional
    shopping day in 2009 - sales grew by approximately 3.5 percent.

    Said comScore chairman Gian Fulgoni:

    "Following last year's disappointing performance when sales fell
    by 3 percent, the e-commerce sector saw a positive 2009 holiday
    shopping season with sales up by 5 percent. Online sales growth
    this year was driven by a continued increase in the number of
    people buying online, but consumers' economic challenges
    resulted in a slight decline versus last year in the amount
    spent per buyer."

    In terms of individual product categories, consumer electronics
    saw particularly strong sales growth of slightly over 20
    percent, while jewelry and watches also turned in a strong
    performance, but which followed a very weak 2008 season.

    "Google Loses Domain To Groovle"
    --------------------------------

    Just before the new year dawned, Google lost its second-ever
    domain name dispute (out of 65!) when the National Arbitration
    Forum dismissed the company's claim that it was entitled to the
    domain name, Groovle.com.

    Google had claimed that the domain name Groovle.com was
    'confusingly similar' to its Google trademark. The ICANN
    accredited National Arbitration Forum, however, disagreed in a
    unanimous decision.

    The three person panel - comprising two retired American judges
    and an American law professor - ruled that Groovle.com was NOT
    'confusingly similar' to Google. In so doing, the panel found in
    favor of the two Groovle owners, young Canadian entrepreneurs
    Jacob Fuller and Ryan Fitzgibbon. Friends from high school,
    Fuller and Fitzgibbon launched Groovle.com in 2007 to be a place
    where people could upload photos and customize their Internet
    start page.

    When commenting on the decision, Fitzgibbon made the interesting
    comment that since they launched Groovle, "Google, Bing and
    Ask.com have each come out with something similar."

    Hmmm... given the likely competition from such behemoths, maybe
    Google's domain name dispute has actually been a marketing boon
    to Groovle, which has obviously received a lot of publicity from
    the domain name dispute alone.

    As Groovle's domain name lawyer and Internet law expert, Zak
    Muscovitch, said: 'Google clearly miscalculated here.' But not
    just from a domain name standpoint – also from a marketing
    standpoint.

    There are two core principles behind this idea - or should I
    say, reality:

    1. The more things you try, the more results you'll get - good
    and bad.

    Unless you can predict the future, you can never really know
    that something - e.g. a business - is going to pay off.
    Therefore, you have to try a lot of things. The more things you
    try, the more 'results' you'll get. For many business owners a
    pattern starts emerging where, out of 10 projects, a few will
    fail miserably, a few will return mediocre results, and a few
    will succeed spectacularly.

    Depending on how uncertain is the field you're in (futures
    trading, making movies and developing pharmaceuticals being
    among the most uncertain), you may well find that half or even
    most of your initiatives tend to fail. But the reality is that
    you have to try a lot of things to yield the winners.

    2. Cut your losses short and let your winners run.

    It doesn't matter if most of your initiatives are failures or
    mediocre performers... as long as you stop investing in the
    losers and poor performers - i.e. cut your losses short - and
    pour all your remaining resources into the winners, i.e. let
    your winners run.

    Let me prove this to you with some simple math.

    Imagine you have $100 to invest in 10 different projects. You
    start off by allocating $5 to each. After a short while, it
    turns out that nine of the projects are losers, which means you
    lose $5 per project and $45 in total. One of these projects is,
    however, a huge success and gives you a 400 percent return on
    investment (ROI), making you $25.

    So where are you at? You've made $25 and lost $45, resulting in
    a net loss of $20.

    Ahhh, but you've still got another $50 to invest, haven't you?
    Where do you put that money? In the one project giving you a 400
    percent ROI, of course! As a result of doing that, you put in
    $50 and make a whopping $250.

    Which means, in total, you've made $230 on your initial
    investment of $100 - a 130 percent ROI overall.

    This isn't quite as good as if you put all your money into the
    single winning project, but the reality is you that never knew –
    and were never going to know - which of the 10 projects was
    going to win, let alone win big.

    In real life, you probably do have a slightly better inkling
    about which businesses, products, services, etc are likely to
    pay off. Plus, things won't necessarily be as simple or as
    straightforward as in this example. But you can see the power of
    cutting your losses short and letting your winners run (or
    reinvesting in your winners). You don't need to succeed all or
    even most of the time, as long as the magnitude of your
    successes far outweighs the magnitude of your failures.

    "Social Media Best For Lead Generation, Not Sales"

    According to a survey from Citibank, the majority of small
    businesses don't believe social networks have helped them grow
    their businesses. On the other hand, the 'Small Business
    Marketing Forecast 2010' from Ad-ology indicates that social
    networking HAS helped with lead generation.

    In Ad-ology's survey of small businesses, one-half of
    respondents cited lead generation as the biggest benefit of
    social networking. They also regarded participating on social
    networks as a good way to keep up with their industries, and to
    monitor online chatter about their businesses.

    Small businesses identified Facebook as the most beneficial
    social networking site, with one-third of respondents
    designating it at least somewhat helpful. Small businesses were
    also more likely to use Facebook over any other social network.

    Meanwhile, 21 percent of small businesses regarded LinkedIn as
    beneficial, whilst 19 percent considered Twitter beneficial.

    Small businesses also attributed a lack of knowledge about
    social networking as a major obstacle to doing more in social
    media. One-half of respondents wished they knew more about the
    channel.

    The biggest obstacle to increasing their social networking
    activities was, however, the view – held by 31 percent of small
    businesses – that 'our customers do not use social networks.'
